If it started within days of returning, is watery, and is improving — hydration and time. If it has lasted beyond a week, if there is blood or fever, if you are losing weight, or if you are immunocompromised, pregnant or elderly — test now. Go to an emergency department, not a lab, if you have: bloody diarrhea with fever, severe abdominal pain, signs of dehydration, a rigid abdomen, or you are an infant, elderly, pregnant or immunocompromised and deteriorating.

FAQ
- Should I use loperamide?
- It can be reasonable in mild non-bloody diarrhea, but it is avoided in bloody diarrhea and suspected STEC. Ask a physician.
- What if others on my trip are sick too?
- A shared source is likely, and identifying the pathogen helps everyone. Come in.
- Do I need a stool culture as well?
- Sometimes, for public-health reporting. We will tell you.
- Can I be treated the same day?
- Yes, if treatment is indicated.
