Fecal elastase-1 estimates exocrine pancreatic function. A low value suggests exocrine pancreatic insufficiency (EPI) — the pancreas is not producing enough digestive enzymes, which causes greasy, floating, foul-smelling stool, bloating and weight loss. The classic pitfall: a watery specimen dilutes the enzyme and yields a falsely low result. Collection quality is part of the test.

Interpreting it honestly

EPI is associated with chronic pancreatitis, pancreatic surgery, cystic fibrosis and diabetes among other causes. The test suggests the state; it does not explain why — and the why is a physician's job.
