When the infection ascends from the bladder to the kidney, the picture changes: fever, chills, flank or back pain, nausea and vomiting, and often feeling systemically unwell rather than just uncomfortable when you urinate. Acute pyelonephritis can progress to sepsis. It is not managed by picking a stronger pill at a walk-in counter.

Go to an emergency department, not a lab, if you have: fever with flank or back pain, shaking chills, persistent vomiting, or new confusion in an older adult. Those point to a kidney infection or sepsis and need urgent clinical care now.

- Can a kidney infection be treated with oral antibiotics?
- Sometimes, in selected stable patients — that is a physician's judgment after examining you, not a website's.
- How fast can this get dangerous?
- Sepsis can develop quickly. If you have fever and flank pain, do not wait for a lab result.
- Is testing still useful?
- Yes — identifying the organism and resistance genes matters. But it happens alongside urgent clinical care, not instead of it.
- What if I am pregnant?
- Pyelonephritis in pregnancy is an obstetric emergency. Go to an emergency department now.
