When the infection ascends from the bladder to the kidney, the picture changes: fever, chills, flank or back pain, nausea and vomiting, and often feeling systemically unwell rather than just uncomfortable when you urinate. Acute pyelonephritis can progress to sepsis. It is not managed by picking a stronger pill at a walk-in counter.

Go to an emergency department, not a lab, if you have: fever with flank or back pain, shaking chills, persistent vomiting, or new confusion in an older adult. Those point to a kidney infection or sepsis and need urgent clinical care now.

If you have fever with flank pain, we will tell you to go to an ER rather than take your money. That is not a disclaimer — it is the correct clinical advice, and we would rather lose the sale.

We name drugs, never doses. Treatment statements follow IDSA, AUA and ACOG guidance; dose and duration are a physician's decision.
Same day, start to finish. Sample by 1:00 PM → results at 4:30 PM → if treatment is clinically appropriate, a licensed physician in our partner network sees you between 4:30 and 6:00 PM, a few minutes away. Sometimes, in selected stable patients — that is a physician's judgment after examining you, not a website's.
How fast can this get dangerous?
Sepsis can develop quickly. If you have fever and flank pain, do not wait for a lab result.
Is testing still useful?
Yes — identifying the organism and resistance genes matters. But it happens alongside urgent clinical care, not instead of it.
What if I am pregnant?
Pyelonephritis in pregnancy is an obstetric emergency. Go to an emergency department now.
