It means neutrophils are in your urinary tract — your immune system is responding to something. It does not tell you what. Pyuria occurs with bacterial UTI, but also with STIs, interstitial cystitis, stones, tuberculosis, and after instrumentation. And pyuria with no symptoms, especially in older or catheterized patients, is common and usually not a reason for antibiotics.

"Sterile pyuria" — white cells with no growth — is a classic prompt to look for an STI or a non-infectious cause rather than to prescribe another antibiotic.
