Herpes presents as grouped vesicles on an erythematous base: a tight cluster of small clear blisters on red skin, which ulcerate and crust. Shingles also blisters — but it follows a nerve band on one side of the body and does not cross the midline. Contact dermatitis is itchy and diffuse. Folliculitis centres on hair follicles.
Have a sore right now? Swab it today while it is fresh — lesion PCR is the most reliable herpes test there is, and it loses sensitivity as the sore crusts over.

Herpes vs. the four rashes it is mistaken for
| Condition | Pattern | Key difference |
|---|---|---|
| Herpes (HSV) | Tight cluster of blisters on red skin | Recurs in the same place; preceded by tingling |
| Shingles (VZV) | Blisters along a nerve band, one side only | Does not cross the midline; often severe nerve pain. Needs urgent care — antivirals are time-critical |
| Contact dermatitis | Diffuse, itchy, follows exposure | No blister cluster, no prodrome |
| Folliculitis | Bumps centred on hair follicles | Pustules, not clear vesicles |
| Syphilis rash | Non-itchy, may involve palms and soles | Blood serology, not a swab — syphilis testing |

See a clinician urgently if the rash follows a band on one side of your body (possible shingles — antiviral timing matters), if it involves the eye, or if you have fever with a spreading rash.
Do not accept an HSV IgM blood test. CDC does not recommend it. If there is a sore, swab the sore. See lesion PCR vs. antibody.
We name drugs, never doses. Antiviral therapy follows CDC guidance; what you take is decided by a licensed physician.
The single most useful thing on this page. If you have a sore right now, get it swabbed today — lesion PCR identifies HSV and types it. Sensitivity falls as the sore dries. Sample by 1:00 PM → result at 4:30 PM → licensed partner-network physician 4:30–6:00 PM. Same day. Walk in, no appointment, no insurance. Text (713) 832-8892.
FAQ
- Can herpes appear on the body, not just the mouth or genitals?
- Yes — buttocks, thighs, fingers (herpetic whitlow). The pattern of grouped vesicles stays the same.
- Could this be shingles?
- If it follows a nerve band on one side, possibly — and that is time-sensitive. See a clinician today.
- Do I need a blood test?
- If there is a lesion, swab it. That answers the actual question.
- How fast is the result?
- Same day at 4:30 PM. Walk in, or text (713) 832-8892.
The sore will crust over in a few days — and the test gets less reliable as it does.
