Small, clear, clustered fluid-filled blisters on a reddened base, usually at the lip border and usually preceded by tingling — that is the classic picture of HSV-1. The alternatives look different: milia are hard and white, mucoceles are single and translucent, allergic reactions swell diffusely rather than forming a tight cluster.

The one detail that separates herpes from nearly everything else: it is a cluster. A single isolated bump with a white head is almost always something else.
